The early signs of hand, foot, and mouth disease (HFMD) develop day by day as follows:

  • Incubation period lasts about 3 to 6 days after exposure to the virus.
  • Day 1-2 (Initial symptoms stage): Children typically develop mild to moderate fever (around 37.5-39°C), sore throat, fatigue, decreased appetite, and sometimes vomiting or diarrhea. These symptoms resemble a common cold or flu and may be accompanied by drooling or discomfort when swallowing.
  • Day 2-3: One or two days after the fever begins, painful mouth sores or ulcers appear inside the mouth, often on the tongue, gums, and inside cheeks. These start as small red spots and then blister, causing pain during eating or drinking.
  • Days 3-5: A rash appears, typically on the palms of the hands, soles of the feet, and sometimes on the buttocks, legs, and arms. The rash consists of flat or raised red spots that may develop into blisters with clear fluid. The rash is generally not itchy.
  • Days 5-7: Symptoms usually peak and then gradually improve. Mouth sores and rash begin to heal, sometimes followed by peeling of the skin on the hands and feet.
  • Most symptoms resolve within 7 to 10 days without treatment.

Key early signs to watch for daily are fever and flu-like symptoms first, followed by painful mouth sores and the distinctive rash on hands and feet shortly after.
