Philip Seymour Hoffman died on February 2, 2014, at the age of 46, from an accidental overdose caused by "acute mixed drug intoxication." The toxic combination included heroin, cocaine, benzodiazepines, and amphetamines. He was found unconscious in the bathroom of his Manhattan apartment with a syringe still inserted in his left arm. Police also found numerous bags of heroin and used syringes at the scene, indicating a relapse into drug use after a period of sobriety.
