To apply for a passport, you need to provide the following documents:

Proof of U.S. citizenship: This can be a U.S. birth certificate, Consular Report of Birth Abroad or Certification of Birth, Certificate of Naturalization, or Certificate of Citizenship. The document must be an original or certified, physical copy with the seal or stamp of the official issuing authority.

Photo ID: A driver’s license, government-issued ID, military ID, Certificate of Naturalization, or previously issued passport (if you are applying for a new one) are all acceptable forms of ID.

A photocopy of both your citizenship document and photo ID .

Passport photo: You must provide one photo with your application.
In addition, you must submit the application form and pay the passport fees. If you are under 16 years old, you must also provide documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport.
