To apply for a US passport, you will need to present a physical, government-issued photo identification document (ID) when submitting Form DS-11. The ID must readily identify you and include your photo, full name, date of birth, and the document issuance date. The following are acceptable primary IDs:
- Valid or expired, undamaged U.S. passport book or passport card
- In-state, fully valid drivers license or enhanced drivers license
- Certificate of Naturalization or Citizenship
- Government employee ID (federal, state, or municipal)
- U.S. military ID or military dependent ID
- Valid foreign passport
If you cannot present one of the primary photo IDs above, you must present at least two secondary IDs. Acceptable secondary IDs include:
- Social Security card
- Voter registration card
- Employee ID
- Student ID
- Health insurance card (not Medicare)
- Expired drivers license or ID card
If you do not have a valid ID, someone may make the request on your behalf. The individual making the request must be a family member or friend who has known you for at least five years and must include in the request:
- A notarized statement from you authorizing the individual to obtain the record
- A notarized statement from the individual stating that he or she has known you for at least five years
- A copy of the requesters valid photo identification
In addition to a photo ID, you will also need to provide proof of citizenship and a passport photo. Proof of citizenship can be a US birth certificate, Consular Report of Birth Abroad or Certification of Birth, Certificate of Naturalization, or Certificate of Citizenship. If you dont want to submit a photocopy of your citizenship evidence, you may submit a second certified copy of your citizenship evidence, which will be kept by the passport agency.
