MDR stands for Member Data Record and it is a document issued by the Philippine Health Insurance Corporation (PhilHealth) that contains a members basic information, details about their current employer (if employed), and dependents or beneficiaries under their name. The MDR is a requirement when processing for PhilHealth during hospitalization and is also one of the major requirements when availing of PhilHealth benefits. The MDR can be obtained online through the PhilHealth website by registering for an account and clicking on the "Print MDR" button on the member information page. It can also be obtained through a walk-in application at any PhilHealth branch by filling out a PhilHealth Request Form and presenting a valid ID to the PhilHealth officer. Employed members can also receive their PIC and a copy of their MDR through their companys Human Resource Department when PhilHealth registration is facilitated by the employer.
