A statutory form is a form created by a government, usually designed to serve as a model form or a free form for the public. Statutory forms can be used for various purposes, such as designating an agent who is empowered to take certain actions regarding your property, or filing information on business activities for registered companies. In the context of government, a statutory form of government is the general form of government of municipalities if the electorate has not adopted, by vote, one of the other forms. Statutory forms can also refer to the form on which a policy is delivered or issued for delivery by the issuer.
