There are several medications that can lower sperm count. Here are some of them:

Alpha-blockers: These medications are used to treat urinary symptoms caused by an enlarged prostate. Silodosin and tamsulosin (Rapaflo® and Flomax®, respectively) can both cause a sharp decrease in the volume of ejaculation, or they may inhibit ejaculation completely. Up to 5% of people who take this medication for either reason will have a reduction in their sperm numbers.

Antidepressants: Tricyclic antidepressants and selective serotonin reuptake inhibitors (SSRIs), such as clomipramine and paroxetine, can lead to significant but reversible decreases in sperm count and motility.

Chemotherapy medications: Chemotherapy drugs can damage sperm-producing cells, leading to a temporary or permanent decrease in sperm count.

Opioids (narcotics): Opioids can decrease testosterone levels, which can lead to a decrease in sperm count.
Its important to note that not all medications that affect male fertility necessarily lower sperm count. For example, some medications can affect sperm motility or morphology. Additionally, some medications can actually increase sperm count, such as estrogen receptor blockers like clomiphene citrate, which stimulates hormone production and can increase sperm production.
