To apply for a passport, you will need to bring the following items:
- Original proof of U.S. citizenship, such as a U.S. birth certificate, Consular Report of Birth Abroad or Certification of Birth, Certificate of Naturalization, or Certificate of Citizenship.
- An acceptable photo ID, such as a valid drivers license, government ID, or military ID.
- A photocopy of both your citizenship document and your photo ID.
- A completed DS-11 application form.
- For children under age 16, documentation that lists the parent(s) or legal guardian(s) of the child applying for a passport, such as a U.S. birth certificate.
- For children under age 16, both parents or guardians must be present with the child when applying for a passport.
It is also recommended to bring a second certified copy of your citizenship evidence or a photocopy of your identification documents to avoid delays in processing.
