Derek Shepherd dies in Grey's Anatomy in Season 11, Episode 21, titled "How to Save a Life." In this episode, he dies from a brain injury sustained after a car accident. The accident occurs as he is on his way to Washington, D.C., where he was going to resign from a job. Although he initially helps others involved in the accident, he is later hit by a semi-truck. Derek is taken to a small hospital that lacks proper trauma care, and due to medical errors including not performing a timely head CT scan, he is declared brain dead. Meredith Grey, his wife, ultimately decides to take him off life support. This episode originally aired in 2015, marking a major and emotional turning point in the series.
