Decorticate posturing is caused by severe damage to areas of the brain including the cerebral hemispheres, internal capsule, thalamus, or the midbrain. It indicates dysfunction in the nerve pathways, particularly involving the corticospinal tract and the red nucleus. This posturing occurs due to lesions above the red nucleus that lead to disinhibition of the rubrospinal tract, causing flexion of the upper limbs and extension of the lower limbs. Common causes include traumatic brain injury, bleeding in the brain, brain tumors, stroke, infections, poisoning, increased intracranial pressure, and brain damage from lack of oxygen or metabolic issues such as liver failure or Reye's syndrome.
